Enrollment Guide 2009-2010

ll employees will have an opportunity to choose the health plan that best suits their needs during open enrollment from May 4 through May 20, 2009.

What's Changing?

STATE BENEFITS

The Urgent Care copay for Blue Care and Aetna HMO plans has been reduced to $20.
The copay on diabetic supplies through Medco Prescription has been reduced to $0.

DISTRICT BENEFITS

Vendor information and forms will be available upon request in your school/main office.

District Dental (MetLife) and District Vision (VBA)

Dependent coverage will now follow the State of Delaware's dependent eligibility guidelines.
Dependents are eligible through the end of the calendar year in which they reach 21 (or the end of the month they reach 24, if a full-time student).

District Prescription (Aetna)

Dependent eligibility will NOT CHANGE, dependents will be eligible for coverage through June 30th of the year in which they reach age 19.
Annual maximum amount will decrease to $1,500.00 per person per year due to vendor cost increase.

District Long-Term Disability
(The Hartford-180/90 day buy-up option)

Changes will not be available online. Please contact the District Benefits Office to make plan changes.
