What Happens When...?
You End Employment

The chart below illustrates what happens to your benefits when you end employment or retire:

Plan	What You Need To Do
Dental	You can continue coverage for up to 18 months by paying the full cost plus 2%.
Prescription Drugs	You can continue coverage for up to 18 months by paying the full cost plus 2%.
Vision Care	You can continue coverage for up to 18 months by paying the full cost plus 2%.
Life and Accidental Death & Dismemberment	Your life and AD&D benefits end. Life insurance benefits may be converted to an individual policy within 30 days of the date you end employment.
Long-Term Disability	Your LTD coverage ends


What Happens When...?
	You Are Hired
	You Get Married or Have/Adopt a Child
	You Become Disabled
	You Die While Employed
	You End Employment or Retire
